This is where to record your visual observations such as posture, gait, breathing pattern, skin discoloration, etc…

If your SOAP Notes include a drawing of a person, you should be able to record most of your visual observations directly on the drawing.

If you observe symptoms that make you think your patient might be victim of domestic violence, consult the following resources:
