SOAP Charts

WinCity Massage SOAP Notes is designed for Massage Practitioners who need to write extensive documentation.

WinCity Massage SOAP Notes is appropriate for you if any of the following applies to you:

•  Some of your clients have health problems or symptoms that they expect you to address.

•  Doctors refer clients to you for treatment.

•  You or some of your clients are seeking insurance reimbursement.

•  The massages you provide vary from individual to individual and are based on your clients report and your findings.

•  Some of your clients consider their treatment to be a regular part of a health care routine.

This type of massage is sometimes called Medical Massage, Therapeutic Massage, Injury Rehabilitation, or Treatment Work.

Wellness Charts

The type of massage that never directly address specific health conditions, symptoms and findings is sometimes called Relaxation Massage or Wellness Massage.

Even if you never directly address specific health conditions, symptoms and findings, you still need to document your sessions to show that your massages are appropriate given your clients' health condition. You can however use a simplified chart such as a HxTxC or Wellness chart.

WinCity Massage SOAP Notes does not offer a HxTxC or a Wellness chart.  If you decide to use WinCity Massage SOAP Notes to chart your relaxation massages, consider filling in only the following sections of the SOAP form:

•  The Session Length and Client Focus sections of the Session Information tab.

•  The Health Concerns, Medications and Symptoms sections of the Subjective tab.

•  The Palpation tab.

•  The Modalities tab.

•  The Homework section of the Plan tab.

These sections correspond to what you would find on HxTxC charts.
